UITH School of Health Information Management ND/HND Application form, 2026/2027


Applicants are hereby invited from suitably qualified candidates for admission into the National Diploma (ND) andHhigher National Diploma (HND) programme, (full-time) of the School of Health Information Management for the 2026/2027 session.

Admission Requirement /Educational Qualification for National Diploma (ND)

  • Candidate must have written 2026 JAMB with the following subject combination ENGLISH LANGUAGE, BIOLOGY, PHYSICS,CHEMISTRY.
  • Minimum of five (5) in English language, Mathematics, Physics, Chemistry and Biology at not more than two sittings.
  • NABTEB is accepted but cannot be combined with any o level certificate.
  • Candidate must not be less than 16 YEARS as at the time of application.
  • Candidate must not have criminal record
  • Candidate must not have deformity of the hands

Admission Requirement/ Education qualification for Higher National Diploma Programme (HND)

  • Applicant must meet the requirement for National Diploma Admission. He/she must possess National Diploma certificate or Health Record Technician certificate.
  • Candidate with ND certificate in Health Information Management must have evidence of 1 year industrial training and should possess professional license certificate{Part I} by the Health Record officers Registration Board of Nigeria (HRORBN)
  • Candidate must also possess the WEST AFRICAN School certificate/General certificate of Education SSCE/NECO/CE with five(5) passes at credit level obtain at not more tan two(2) sittings and NABTEB in one (1) sitting. This must include English language, Mathematics, Physics, Chemistry and Biology.
