• Read the requirement for each courses before choosing course/programme of choice. The college is not liable (or candidates’ (allure to read and know the requirements for each courses they make their choices.
• Ensure that all spaces are correctly filled before the online submission of the form
• The College is not liable for candidates’ failure fill correctly all spaces before the online submission of the form.
• Candidates are expected to print out an acknowledgement/Identification card to be presented on the day of the entrance examination.

Candidates are to obtain Pin Voucher from any Zenith Bank branch in Nigeria, on payment of a non-refundable fee of Nine thousand, five hundred naira (N9, 500) only and log on to the School website www.deltashtportal.com  to register online.

For E-pin Option: Enter your Admission Pin you purchased from hank and click on “proceed’ to fill and complete admission Form.

For E-payment option: click on proceed, fill the admission form and make payments online using ATM Card or 10 digit account Number.

Candidates are expected to print out an acknowledgement/identification card to be presented on the day of the entrance examination.

ENTRANCE EXAMINATION DATE

The entrance examination holds on Friday June 18th 2021 at the College of Health Technology. Ofuoma premises at 7:30am prompt.

The following are the items to come with:

a. Original and photocopy of credentials
b. SSCE, NECO OR WAEC
c. Local government of origin
d. Birth cetificate or age declaration
e. Two(2) recent passport photograph
f. Affidavit of change of name
g. A copy of the online registration print out
h. Letter of attestation
i. Writing materials
j. No calculator or handset is allowed into the exam hall.

Closing date for submission of application: Wednesday, 16th June. 2021
